Exercise physiology on a GP care plan If you live with a chronic condition, your GP may be able to put you on a GP chronic condition management plan. It lets Medicare pay a rebate toward up to five allied health visits each calendar year, including exercise physiology at ACE Physiology & Rehab in Wantirna. All five can be with an exercise physiologist, or shared across a care team, as your GP decides. You pay a $54 gap per visit.

Common conditions we see on care plans: type 2 diabetes, osteoporosis and osteopenia, neck, back and knee pain, hip and knee replacements, high blood pressure, high cholesterol, Ozempic and Mounjaro users, and fatty liver (NAFLD).
